How to Get a Maryland Death Certificate

Requests for certified Maryland Death Certificates may now be made on a walk-in basis in Prince George's County at the Office of Vital Records, Prince George's County Health Department in Landover, Maryland within 30 days of the date of death.

Note: The Certificate may be available if originally filed in Prince Georges County. Although the death occurred in Prince George's County, it may not be on file with the Prince George's County Health Department Vital Records Office.

Who Can Request a Copy

You may obtain a Maryland Death Certificate if you are:
  • A funeral director
  • An immediate relative of the deceased
  • A beneficiary of the deceased
  • A person of tangible interest

Copy Fee

There is a $25 fee for the first Certified Death Certificate copy and $20 for each additional copy purchased the same day. Cash, check, money order, Credit or Debit Cards (Visa, Discover, MasterCard) are accepted as payment.

  • A $2.50 service fee applies to Credit/Debit card transactions.
  • Check or money order should be made payable to: Prince George’s County Health Department

Certificate Form

Prior to coming to the Medical and Vital Records office, print the Request for a Copy of a Death Certificate Form (PDF). Bring the completed form and your payment during office hours.

Maryland State Archives

For certificates not filed with the Prince George's County Health Department Vital Records Office, or for requests made 31 days after the date of death, will need to submit request for a Maryland Death Certificate directly from the Maryland State Vital Records Office by going to: Maryland State Archives or call 410-764-3053.
